Breast cancer management has evolved significantly over the years. Today, surgery is tailored not only to remove cancer effectively but also to preserve body image and quality of life. 

• Invasive breast cancer
• Ductal carcinoma in situ (DCIS)
• Locally advanced breast cancer
• Recurrent breast cancer
• High-risk breast lesions (ADH, LCIS)
• Prophylactic surgery for BRCA mutation carriers 

1. Breast-Conserving Surgery (Lumpectomy)

Also known as wide local excision, this procedure removes the tumour with a rim of normal tissue while preserving the breast. It is typically followed by radiation therapy.

Ideal for:

• Early-stage breast cancer
• Single, localized tumours
• Patients desiring breast preservation

2. Mastectomy

Removal of the entire breast tissue when breast conservation is not suitable.

Types include:

• Simple (Total) Mastectomy
• Skin-Sparing Mastectomy
• Nipple-Sparing Mastectomy
• Modified Radical Mastectomy

Reconstruction options are discussed in detail to restore natural contour and symmetry.

• Modified radical mastectomy
• Sentinel lymph node biopsy
• Oncoplastic breast surgery

3. Sentinel Lymph Node Biopsy (SLNB)

A minimally invasive technique to assess whether cancer has spread to axillary lymph nodes. This reduces the need for full axillary dissection and lowers the risk of lymphedema. 

4. Axillary Lymph Node Dissection

Performed when there is confirmed nodal involvement. Care is taken to minimize complications such as arm swelling and shoulder stiffness. 

5. Oncoplastic Breast Surgery

A specialized technique combining cancer removal with plastic surgery principles to maintain breast shape and symmetry. Particularly useful in larger tumours relative to breast size. 

 Neoadjuvant Therapy Integration

For selected patients, chemotherapy or targeted therapy is administered before surgery to:

• Shrink tumours
• Allow breast conservation
• Assess tumour response

Management of BRCA & High-Risk Patients

Risk-reducing mastectomy and surveillance strategies are offered in consultation with genetic counselling teams. 

• Implant-based reconstruction
• Autologous tissue reconstruction
• Immediate or delayed reconstruction
• Contralateral symmetrisation procedures
